For Referring Physicians


We accept the following patients:

  • Orthopedic: total joint replacements (including bilateral knee replacements), fractures, falls, trauma
  • Post-surgical: cardiac, vascular, plastic, spinal, complex GI, complex drains
  • Neurological: CVA, Parkinson’s, multiple sclerosis
  • Cardiopulmonary: heart failure, COPD, interstitial lung disease
  • Complex medical: pneumonia, IV antibiotic therapy, PICC lines, established tracheostomies
  • Complex wound care: including wound-VAC (vacuum-assisted closure) therapy
  • Total parenteral nutrition: to support medical/surgical recovery
  • Chronic kidney disease: outpatient chronic dialysis for patients with acute conditions
  • Oncology: patients undergoing radiation and/or chemotherapy who also need to regain strength
  • Hospice: when rehab goals change, the plan of care can be adjusted to include end-of-life care

We aim to avoid hospital readmissions through:

  • A daily interdisciplinary team meeting that evaluates each patient's progress
  • Collaboration with hospital specialists, support services, and the Rapid Response Team
  • A comprehensive electronic medical record, which allows for improved communication among all members of your healthcare team throughout the continuum of care from acute hospitalization to the TCU, home care, or outpatient rehabilitation
  • Comprehensive, patient-centered discharge planning that begins on the day of admission with a focus on a safe discharge plan home

Health Insurance Coverage

We accept most insurance plans, including traditional Medicare and managed Medicare recipients who qualify for direct admission to the TCU from an emergency department, their physician's office, or home through the Pioneer ACO three-day waiver. Emerson is a preferred provider for the Partners HealthCare and Atrius Health ACOs.
