Infants (0-2 Years) 

  • Newborn Screenings: At birth. 

  • Well-Baby Visits: At 1, 2, 4, 6, 9, 12, 15, 18, and 24 months. 

  • Developmental Screenings: At 9, 18, and 24 or 30 months. 

  • Hearing Screenings: At birth and during regular check-ups. 

  • Vision Screenings: Starting at 6 months. 

  • Immunizations: 

  • Birth: Hepatitis B 

  • 2 Months: DTaP, Hib, IPV, PCV13, RV 

  • 4 Months: DTaP, Hib, IPV, PCV13, RV 

  • 6 Months: DTaP, Hib, IPV, PCV13, RV, Influenza (annual starting at 6 months) 

  • 12 Months: MMR, Varicella, Hepatitis A, PCV13, Hib 

Early Childhood (3-5 Years) 

  • Well-Child Visits: Annually. 

  • Vision Screenings: Annually. 

  • Hearing Screenings: Annually. 

  • Dental Checkups: Every 6 months. 

  • Developmental Screenings: During annual visits. 

School-Age Children (6-12 Years) 

  • Annual Well-Child Visits: Annually. 

  • Vision Screenings: Annually. 

  • Hearing Screenings: Annually. 

  • Dental Checkups: Every 6 months. 

  • Developmental and Behavioral Screenings: During annual visits. 

Adolescents (13-18 Years) 

  • Physical Exams: Annually. 

  • Vision Screenings: Every 1-2 years. 

  • Hearing Screenings: Every 1-2 years. 

  • Dental Checkups: Every 6 months. 

  • Mental Health Screenings: During annual visits. 

  • Immunizations: 

Young Adults (19-39 Years) 

  • Physical Exams: Every 1-3 years. 

  • Blood Pressure Checks: At least once every 2 years. 

  • Cholesterol Screenings: Every 4-6 years. 

  • STI Screenings: Annually for sexually active individuals. 

  • Pap Smears: Every 3 years starting at age 21. 

Middle-Aged Adults (40-64 Years) 

  • Physical Exams: Every 1-2 years. 

  • Blood Pressure Checks: Annually. 

  • Cholesterol Screenings: Every 5 years. 

  • Diabetes Screenings: Every 3 years starting at age 45. 

  • Colorectal Cancer Screenings: Starting at age 45, every 10 years (colonoscopy) or as recommended. 

  • Breast Cancer Screenings: Mammograms every 1-2 years starting at age 40. 

  • Cervical Cancer Screenings: Pap smear every 3 years or Pap smear with HPV test every 5 years. 

  • Bone Density Test: Starting at age 65 for women or earlier if at risk. 

Seniors (65+ Years) 

  • Annual Physical Exams: Annually. 

  • Blood Pressure Checks: Annually. 

  • Cholesterol Screenings: Annually or as recommended. 

  • Diabetes Screenings: Annually. 

  • Bone Density Tests: Every 2 years. 

  • Vision and Hearing Tests: Annually. 

  • Cancer Screenings: As recommended (continue breast, cervical, colorectal, prostate screenings as advised). 

  • Mental Health Screenings: During annual visits.
